To install mouthpiece, insert the star shaped end of the mouthpiece's black piece into the tube until its two notches are even with the opening of the tube as shown in the picture.

Insert the mouthpiece's blue portion over the black piece and tube. The two holes in the black piece should be positioned horizontally for optimal liquid flow.
